Position:
Nurse: Full-time positions available
Description:
Pacific Coast Health is currently looking for a Nurse to join our team as a Client Care Manager. (CCM)
We are currently seeking a Full-Time RN or LPN to provide nursing services to our clients in their homes throughout Victoria and the South Island. Experience in Community Nursing, Case Management, Foot Care or Wound Care is an asset, but we are willing to train the right person.
As a CCM you will visit clients and their families in their homes for assessments and follow-ups, wound care, and other nursing tasks. The CCM also delegates appropriate tasks to unlicensed personnel, assists clients and workers over the phone, and completes funder reports.
CCMs are responsible for carrying out the nursing process utilizing techniques and interpersonal skills that will enhance our clients' quality of life and promote their highest functioning ability. You must be a Positive and professional individual with strong communication skills and the ability to work independently.
Terms of Employment:
This position is Full Time and based out of our office located at 2631B Douglas St, Victoria, BC CCM hours are Monday through Friday from 9 to 5, with rotating on-call as needed.
Responsibilities:

Travel to client homes to provide care utilizing the RN full scope of practice

Perform client Assessments and Case Management
-
Educate clients on the management of chronic diseases

Delegation of appropriate tasks to field staff
-
Ability to work either in a team setting or independently
-
Effectively communicate with all members of the health care team
-
Ensure a responsive and caring approach to all clients and staff
Qualifications:
-
Must have an active license with the CRNBC or the CLPNBC
-
Must possess a valid Driver's License and have access to a reliable vehicle
-
Home Care or Wound Care experience is an asset
-
Foot Care Certification is an asset
-
CPR and First aid is an asset
-
Must have excellent communication skills

Be reliable, confident and accountable

As a Community Health Worker at We Care, you will work in our clients’ homes helping them to live their lives. We are looking for people who can work flexible hours, including mornings, evenings, and weekends. Potential live-in shifts are available if you are interested.
